Hospital
A facility which:
– Is licensed and operated as a hospital in the state in which it is located.
– Operates primarily for the treatment of sick or injured persons as inpatients.
– Provides nursing service 24 hours a day under the supervision of a registered graduate professional nurse.
– Is supervised by a staff of licensed physicians.
– Has medical or surgical facilities or has access to such facilities.

Intermediate Care Facility
A facility which:
– Is licensed and operated as an intermediate care facility in the state in which it is located.
– Provides nursing service 24 hours a day under the supervision of a registered graduate professional nurse or licensed practical nurse.
– Maintains a daily medical record of each patient.

Medically Necessary
Confinement, care or treatment which:
– Is appropriate with the diagnosis in accordance with accepted standards of practice.
– Cannot be omitted without adversely affecting the insured’s condition.

Palliative Care
Care directed at managing the symptoms the hospice patient experiences and intended to enhance the quality of life for hospice patients and their family, but not directed at curing the illness.

Skilled Nursing Facility
A facility which:
– Is licensed and operated as a skilled nursing facility in the state in which it is located.
– Provides skilled nursing a care under the supervision of a licensed physician.
– Provides nursing service 24 hours a day under the supervision of registered graduate professional nurse.
– Maintains a daily medical record of each patient.
